
"Teeth Marks" is mostly concerned with the drug switch. When Shinwell decides not to adjust his medication again, he soon finds himself the object of frightening blackmail, with his adoptive parents' safety at risk if he doesn't comply. He feels stuck and starts to change the tablets as instructed, but the week's patient, Ginny (Caitlin Stasey), who conveniently has episodic amnesia, stops him in her tracks and quickly forgets what she saw. She does, however, record the occurrence in her planner, which starts a chain of events that almost reveals Shinwell and forces Moriarty to use his power in ever-more-nefarious ways.
When Watson takes his tablets, he at least realizes something is wrong and suspects Shinwell, which is to be expected for a character with his intelligence. For that reason alone, this subplot had little potential, so I'm glad Watson was able to put it together so quickly. He asks Stephens to examine his drug, which turns out to be hallucinogenic. There aren't many possible suspects who could have tampered with it, since he is already taking it illegally with Shinwell's knowledge.
